Referring a patient
Thank you for considering Physionorth. We have been treating patients at 69 Eyre Street, North Ward since 2001. Our team includes physiotherapists, an accredited exercise physiologist and remedial massage therapists, and we treat one patient at a time with longer appointments.
We welcome referrals from GPs, specialists and dentists, including referrals under a GP chronic condition management plan.
How it works
Send the referral
Fax 07 4724 0387 or email reception@physionorth.com.au, or give it to your patient to bring.
We contact your patient
If you include their phone number, we can call to book them in.
Assessment
Your patient has a full assessment with a physiotherapist or exercise physiologist.
We report back
With your patient's consent, we send you a letter with our findings and treatment plan.
Helpful details to include
- your patient's name, date of birth and phone number
- the reason for referral and any relevant history
- results of any imaging or tests
- for care plan patients, the number of allied health services referred
- any precautions or surgical protocol
