Privacy policy

How we collect, use and protect your personal and health information.

The Physionorth reception desk on Eyre Street, North Ward
Your records are kept confidential at our clinic on Eyre Street.

Your privacy matters to us

Physionorth complies with the Privacy Act 1988 and the Australian Privacy Principles. This policy explains what personal and health information we collect, why we collect it, and how we look after it.

In short: we only collect what we need to care for you, we keep it secure, and we only share it with your consent or where the law allows.

What we collect

We collect personal and health information that we need to provide your care, such as your name, contact details, date of birth, health fund or Medicare details, medical history, and notes about your assessment and treatment. We collect it with your consent, usually from you directly, or from your doctor or other health professionals when they refer you.

How we use it

  • to assess, treat and care for you
  • to communicate with your doctor and other members of your health care team, where needed
  • to process payments, health fund and Medicare claims
  • to send appointment reminders and confirmations by SMS or email
  • to give you access to our online booking and exercise app

We only use or share your information for these purposes, or for directly related purposes you would reasonably expect, unless you give consent or the law requires it (for example, a court order or debt collection).

Marketing messages

We occasionally email information about promotions, products and services, but only if you have agreed to receive them. You can opt out at any time by using the unsubscribe link or telling reception. You can also ask us to stop SMS appointment reminders.

How we keep it safe

Your details are stored confidentially with your clinical notes. We take reasonable steps to protect your information from misuse, loss and unauthorised access, to keep it accurate and up to date, and to securely destroy or de-identify health information we no longer need.

Accessing and correcting your information

You can ask to see the information we hold about you, and ask us to correct it. Please make your request in writing, addressed to the Practice Director. A fee may apply. In rare cases, for example where access could pose a serious threat to someone's life or health, the law allows us to refuse access, and we will explain why.

This website

When you use the contact or feedback form, we receive the details you enter so we can reply. Please don't send detailed health information through the website. The map on our contact page is provided by Google and only loads if you choose to show it. Online bookings are handled by our booking provider, PracSuite.

Questions or concerns

If you have a question or concern about privacy, contact our Practice Manager on 07 4724 0768 or practicemanager@physionorth.com.au. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner at oaic.gov.au.

We will give a copy of this policy to anyone who asks for it.
